The No Wrong Door service is an innovative and exciting opportunity to provide a bridge between Primary Care, Community Mental Health services and Substance Use services.

For too long, people who have co-occurring conditions have been referred around between services, none of which meets their overall needs for support.

No Wrong Door aims to change this by using a person-centred approach to ensure that appropriate agencies are involved in a person's care when they need it the most.

These vacancies have arisen as a result of an unmet need identified for those members of the public who struggle with the co-occurring conditions of poor mental health and substance use.

Main duties of the job

We are seeking a Full Time (37.5 hours) Co-Occurring Conditions Recovery Worker to join our West Hampshire No Wrong Door Team, working across Eastleigh and the New Forest.
  • Liaising with partner agencies in Primary Care, Community Mental Health care and substance use services
  • Facilitating joint assessments with partner agencies and the individual
  • Formulating joint care plans with partner agencies and the individual
  • Ensuring that people receive the most appropriate care at the most appropriate time
  • Advocating for clients with partner agencies
  • Embracing the concept of recovery and demonstrating this in your clinical work
  • Ensuring that your work is evidence based, up to date and in accordance with current policy and legislation.
  • Keeping up to date and accurate records and documentation.

Detailed job description and main responsibilities

The role lends itself to applicants who have a strong foundation of knowledge and practical experience in Mental Health or Substance use.

We recognise that a hybrid of both skillsets may not be available, and would support the successful candidates in gaining the knowledge they currently lack in the area in which they are currently less experienced.

Delivery of harm minimisation, facilitating safe reduction plans as part of robust care planning, liaison with multiple health care professionals, conducting home visits, and meeting people outside the normal healthcare environment will form part of the role.

In addition to this, the successful applicants will also be required to create holistic care plans, identify and manage risks accordingly. Caseload management skills and good organisational skills are essential to supporting the clients treatment journeys.

A current driving licence and own transport is essential.

Qualifications

Essential criteria
  • Foundation Degree / HNC in related field which may include counselling, social work, nursing, psychology, human resources, social studies
Desirable criteria
  • Relevant training / Qualification in associated field
  • CBT • Motivational Interviewing
  • Brief solution focussed therapy
  • Mapping techniques (ITEP/BTEI)

Experience

Essential criteria
  • Considerable experience of working with service users in the community with drug and alcohol and/or mental health needs
  • A commitment to and understanding of harm reduction and recovery approaches to working with people with co-occurring conditions of mental health and drug and alcohol use.
Desirable criteria
  • Experience of working with multiple agencies in primary and/or secondary care

  • Excellent communication skills and a willingness to promote service user involvement
