This is an exciting time for DCHS and we are looking to recruit a new Occupational Therapist to our Community First team in Derby City

You will be able to develop your assessment, rehabilitation and discharge planning skills to ensure that patients remain safe within their usual place of residence.

Rehabilitation is delivered to patients in community settings with a focus on improving health and well-being, working toward patient centred goals and preventing hospital admission or facilitating discharge home.

Our preceptorship programme has been running for many years and offers monthly protected time for education and support across multiple disciplines.

A network of Clinical Lead therapists provide day to day clinical support and development across the trust and our Professional Lead AHP ensures that AHP’s have a clear voice across the organisation.

You will need to be able to work from different bases and travel to patient homes within the locality and borders and must therefore be able to meet the travel requirements of the post.

Our service is provided 7 days a week between the hours of 8.00am and 6.30pm.

Our Integrated Community Services Teams are enthusiastic with a breadth of experience and a shared goal to offer the highest standard of care to our patients.

As a B5 therapist you will play a key role in delivering high quality care to patients with a range of conditions and needs across a variety of care settings, consolidating your skills to provide a comprehensive assessment, identify clear patient goals and provide a treatment plan often using an MDT approach.

You will have the opportunity to develop leadership skills through supporting other members of the team and students.

Our rapid teams promote flow by preventing hospital admissions, facilitating prompt discharges from hospitals, and supporting our community support beds

You will be responsible for maintaining competency to practice through maintenance of professional registration, Continuous Professional Development activities and maintain a portfolio which reflects personal and professional development and evidence of learning beyond registration

The Purpose and Activities of DCHS?

Our purpose is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire.

We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes and, increasingly, in people’s own homes and via virtual consultations.

During the pandemic response we have led the implementation of public vaccination centres in collaboration with partners across health and social care.

We are looking for a Band 5 Community Occupational Therapists to join our hard-working team.

We are committed and passionate about promoting health, well-being and independence. We are a values-based trust and are looking for a dedicated and enthusiastic clinician who believes in the provision of patient-centred care.

We wish to recruit Occupational Therapists who have a clear understanding of the developing role of the therapist within a community setting, working predominantly with older people.

Candidates should demonstrate the ability to work in a team which delivers innovative, responsive services to meet both the needs of patients and the organisation.

We are committed to staff development and you will have a named supervisor, the support of clinical specialist physiotherapists and occupational therapists and the opportunity to attend courses and training including leadership development.

You will be responsible for the supervision and support of therapy staff and support workers.

The integrated team works closely with health and social care professionals as part of the Community First team. It strives to empower patients, families and carers in both their own home and other places of residence.

You will offer bespoke rehabilitation to the frail and elderly including complex fallers, patients with neurological conditions, complex co-morbidities and orthopaedics.

The therapy service is delivered 7 days a week 8 - 1830 to ensure we are responsive to patient and service needs.

You will be expected to travel regularly throughout the locality of the team and therefore you must have access to your own means of transport (ideally a full UK driving licence and vehicle for business use is required for this post).

This post is appropriate for newly qualified Occupational Therapists. A comprehensive preceptorship and mentoring programme is offered.
