This is an exciting opportunity to join the Sirona Diabetes and Nutrition Services (DANS). We are looking for an enthusiastic, motivated, and innovative experienced Nurse to join our growing team of multi professional staff as a Diabetes Specialist Nurse.

The service is an integrated team with nurses and dietitians working seamlessly together for the service. We work closely with other health providers such as Primary Care, Acute Hospitals, Ambulance service as well as other services within Sirona such as, Integrated Network Teams (INTs), Podiatry and other Specialist Services.

We have a strong focus on collaborative working to enable knowledge and skill development to improve patient health needs and outcomes. To support this, the role will also include the delivery of education sessions to health care professionals and people with diabetes across the region.

Alongside the clinical aspect of this post, there is opportunity to participate in audit, research activities, pathway and policy development, staff, and student training. You will need to demonstrate good leadership and team working skills with a practical, forward-thinking approach to service delivery and improvement.

You will also need to be an excellent communicator with the ability to work in a holistic manner and build relationships with a variety of stakeholders.

The service is committed to Continuous Personal Development and we have good clinical supervision models and inhouse CPD in place to support you.

Main duties of the job
  • Working as part of the Integrated Diabetes and Nutrition Service (DANS) the DSN will work with stakeholders to promote the highest possible quality of life, short and long term for the individual with diabetes and their family/carer.
  • Manage a small caseload of complex diabetes cases, with particular responsibility for challenging clinical situations to reduce hospital admission and optimising patient empowerment strategies and improving quality of life.
  • Support the DANS team to work closely with Primary Care to support their diabetes related training needs e.g. virtual clinics, practice-based training.
  • Act as a source of advanced, evidence-based information for diabetes advice and guidance to support colleagues and other health professionals within the specialist area of diabetes management and contribute to the development of local diabetes pathways.
  • Deliver high quality diabetes training to health professionals and structured patient education
  • Provide clinical advice and guidance (A&G) as part of the services dedicated A&G line for health care professionals
  • To participate in organisational and health related promotional events targeting specific cohorts and populations as identified

Person specification

Experience

Essential criteria
  • Clinical experience of working with people with Diabetes
  • Understanding of the implication of cultural difference for service delivery
  • Able to communicate effectively and clearly in both verbal and written format with patients and other health care professionals to ensure that care, treatment and support are not compromised
  • Ability to work well within a team
  • Developing relationships with external organisations and across boundaries
  • Ability to support development and implement policies and service developments
Desirable criteria
  • Effective training, teaching and presentation skills appropriate for patients, carers and healthcare professionals
  • Experience of working within a multi-disciplinary team
  • Experience of Initiation of insulin therapies.
  • Experience of working in a variety of community settings and with primary care teams

Qualifications

Essential criteria
  • Current registration with NMC or other relevant professional body of registration
  • Evidence of continuing professional development
  • Hold a UK Driving License and have access to a vehicle or transport during working hours.
  • Evidence of up to date knowledge of diabetes management, national and local guidelines and evidence base via current research
Desirable criteria
  • Successful completion of post graduate L6 (or higher) course in diabetes or equivalent or willingness to undertake
  • Independant prescriber or willingness to work towards
  • Accredited module in teaching and assessing qualification i.e. FLAP, SIPP
