Job overview

An opportunity has arisen for a keen, enthusiastic nurse to work as part of Bridgend south CMHT.

The post is at band 6 and, the successful applicant will work with a multi-disciplinary team.

The successful applicant will be the care co-ordinator for a caseload of patients and work within the framework of the Mental Health Measures (2010) Wales providing specialist knowledge and advice to a range of service users and providers.

Main duties of the job

The successful applicant will be responsible for a caseload of patients with varying mental health needs and diagnosis. This will involve a comprehensive assessment of need, care planning and implementation of that care along with regular reviews in line with team and national standards.

As a member of the team, there is a requirement to work weekends on a rota basis. This rota is shared between staff in both north and south CMHT's. Staff are generally required to work approximately 4 weekends per year, 9 - 5 including some Bank Holidays.
