To support the delivery of the new GP contract helping to shape and inform innovative and exciting new ways of working into the area. Continually develop and improve the delivery of care in collaboration with partner providers and other stakeholders to meet the local population health needs.
In supporting the development of the networks, the services required to comply with the DES and the post holder will work closely with and also provide support to the network clinical directors. Working collaboratively to support the transition of PCNs to become part of Integrated Neighbourhood Teams (INT) Working collaboratively to find solutions that impact on System flow, Quality Improvement and Patient flow The post holder will also support the collection of data and information to ensure robust budget monitoring, payments and provision of assurance to the network and/or commissioners relating to DES compliance; and subsequent requirements of INTs.
