This is an exciting opportunity to work in the community within a multi-disciplinary, integrated team in the South Kent Coast and Thanet area.
As a Senior Community Learning Disability Nurse you will be part of a proactive and supportive Learning Disability Nursing Team enabling and facilitating people with a learning disability to meet their health needs. You will facilitate access to mainstream health services, working collaboratively with primary care, acute hospitals, and other community health providers.
You will be actively involved in promoting the individual’s independence and understanding of their health needs, working to reduce health inequalities and implementing reasonable adjustments. You will hold a caseload which you will manage under clinical supervision.
You will have line management responsibilities for band 5 Community LD Nurses/Nursing Support Workers and provide supervision and support the development of other team members, including responsibility for supervision and assessment of Nursing Students/Apprentices on placement with the team.

You will use your clinical judgement to undertake timely evidence-based assessment, planning, implementation and evaluation of holistic nursing interventions on a wide range of needs including end of life care, dementia, sexual health, behaviour, complex care, transition, desensitisation, and health education and promotion including development and delivery of group work.

Main duties of the job

You will need: RNLD/RNMH Qualification, or RMN or RGN.

A minimum of 2 years’ recent post-qualifying experience of working with people with Learning Disabilities
Good organisation, communication and interpersonal skills
To be proactive, dynamic and committed to maintaining a flexible and effective nursing service
Hold a full, clean driving licence and have your own vehicle.
Good up to date knowledge of people with learning disabilities and their associated health needs – this is essential

RMNs and RGNs applying must have demonstrable experience of working with people with a Learning Disability

You will be required to work with people with a learning disability and their support networks, in a variety of locations therefore being a car owner/driver is essential, subject to the provisions of the Equality Act (2010).

Qualifications

Essential criteria
  • You must be a registered nurse RLDN/RMN or RGN (RMN's and RGN's applying must demonstrable experience of working with people with a learning disability

Experience

Essential criteria
  • Minimum of 2 years’ experience in area of practice
Desirable criteria
  • Management or supervisory qualification

SPECIAL KNOWLEDGE AND EXPERTISE

  • Understanding of National Service Frameworks and NICE guidance to relevant to care setting and its application across health care services Evidence of continued relevant professional development and implementation in practice within the last 12 months
