You will be working with patients in their own homes and community bed based settings.
The team adopts a Reablement approach to help them maximise our patient’s independence and make informed choices about their long-term care needs. Our aim is to develop a culture and understanding that being in a home environment is more beneficial for individuals than being in hospital.
We are looking for flexible team players who have a background in care. The role will involve working independently and as a team member. You will need to be happy to work a shift pattern which includes early and late shifts. You will need to be confident, professional and a good communicator.The successful candidates will be working with members of a multidisciplinary team, will need to have an NVQ3 in Health and Social Care and a willingness and ability to travel around the Ipswich and East Suffolk area.
The Therapy Service has a high profile within the Trust and thrives on innovation and challenges. The service has taken a lead role in many developments, including reablement, Discharge to Assess, REACT and Frailty Assessment Base, which is helping to shape the future of patient pathways within the local area.
Please be aware that this role require full flexibility including night sits (unlessexempt).
