Conduct comprehensive assessments of clients' mental health status, including evaluating symptoms, behaviours, and functioning levels.Develop individualized care plans in collaboration with clients, their families, and other healthcare professionals to address specific mental health needs and goals.
Coordinate care and services among various providers, ensuring clients receive comprehensive support and follow-up care.
Qualifications and Requirements:
Must hold a mental health nursing degreeFully enhanced valid DBS
Car driver is essential
