Job overview

The Emergency Department is looking for a highly-motivated, reliable and enthusiastic individual to join the team as a Patient Tracker.

Your role will be to ensure accurate, timely data input and flow support is provided to the wider team to prevent unnecessary delays and improve patient experience. You will also escalate potential delays in a timely manner, working closely with doctors, nurses and support staff.

Main duties of the job

To work primarily independently and on own initiative with minimal supervision to carry out the duties of the post.

Take responsibility for the prioritisation of workload. Respond to changing priorities and conflicting demands in an effective manner and develop systems to improve effectiveness of the post.

The role involves long-day working (i.e. 07:00-19:15 or 19:00-07:15). This will include days/nights, weekends and bank holidays.

To actively monitor and manage the pathway of patients in the Emergency Department to ensure that the patient journey is managed within the relevant time-scales.

Review and track patient information and length of time in the department on the ED IT system (Symphony) ensuring that any delay in the pathway is escalated in line with agreed protocols.

Detailed job description and main responsibilities

ROLE AND RESPONSIBILITIES

Work alongside the ED Manager and Nurse in Charge to ensure investigations and subsequent transfers from the ED department are arranged effectively to achieve national and internal targets.

To be mindful of the access time target to include criteria for 4hr and 12hr DTA patient failures and ensuring that reasons for them are clearly and accurately communicated and documented.

Liaise with multidisciplinary teams across the Trust and interpret and convey information accurately and concisely.

Expedite admissions for patients if necessary through negotiation with the clinician and the Capacity Coordinators/Site Manager.

Take responsibility for ensuring that the site manager is aware of the changing pressures within the department and capacity requirements.

To maintain up to date knowledge of all targets and processes relating to the patient pathway and to be a point of reference within the department for other members of the MDT and to ensure we are maintaining Professional standards.

Take responsibility for ensuring that results from investigations are obtained and reports made available to the clinician in a timely manner thus ensuring that discharges/decisions happen in a timely manner

To rotate within the urgent care directorate as necessary.

Work collaboratively with all disciplines in all directorates across the Trust.

To be responsible for extracting clinical data from medical records entering detail onto computer system to ensure that the correct tariff is applied.

Tracking diagnostic requests and results and informing clinicians.

Ensure clinical system symphony is kept up to date at all times.

Ensure colleagues are aware of how to correctly record data on the symphony system so as to reflect an accurate picture of patient pathways.

To support the clinical team to ensure all paperwork is complete and copied to expedite admissions for patients.

To inform the Nurse in charge, Directorate Manager and site team of any delays in Ambulance handover times and record details accordingly.

Support ward staff with maintaining patient flow and any details and records.

To attend and participate in loggist training/meetings.

Educations, Training and Qualifications

Essential criteria
  • Minimum 3 GCSE at Grade C or above including English or equivalent.
  • European Computer Driving Licence or equivalent knowledge.
Desirable criteria
  • Educated to A level, NVQ Level 3 or equivalent level of knowledge or experience.
