Navigator- Social Worker/Mental Health Professional | Hampshire Hospitals NHS Foundation Trust
This post is based within Hampshire Hospitals Foundation Trust Infected Blood Psychological Service (IBPS) hosted by the Haemophilia and Thrombosis Centre. The service provides multidisciplinary care for those infected and affected by the contaminated blood products by a) conducting outreach and psychosocial assessments led by a navigator; and b) providing psychological therapies by a team of psychological professionals.
The post holder will provide a high-quality navigation/care support service, which includes psychosocial assessments and interventions to beneficiaries of the Infected Blood Psychological Service (IBPS).
The post holder will lead on outreach programmes and support the overall development of the newly founded service.
The post holder will be essential in delivering a multi- disciplinary approach to supporting the beneficiaries of the service, and the HCCC.
The navigator element of the service will be provided on a large geographical area.

- Assess and evaluate beneficiaries’ emotional, social, practical, and financial needs in collaboration with the specialist multi-disciplinary team, and support or signpost accordingly E.g., advising on housing, finances, and benefits problems at work and atschool.
- Respond independently to crises in conjunctionwith medical staff, IBPS psychological professionals, and the multi-disciplinary team.
- Liaise and provide specialist knowledge on the social needs of the Infected Blood community to both statutory and non-statutory agencies within the hospital and thecommunity.
