Job overview

Clwyd Ward is looking for an enthusiastic and motivated individual to join our team. We currently have a Ward Clerk vacancy which is a permanent post for 16 hours per week, including alternative Saturdays.

We require the successful post holder to be proactive, organised with good time management skills, within our busy, friendly and hard working team.

Clwyd Ward is dedicated to MSK Orthopaedic/Enhanced Recovery patients. The ward is led by Ward Manager Caroline Morris, Sisters Jayne Long and Louise Beaumont and specialises in the care of patients post surgery mainly hip and knee replacements.
Clwyd ward is a busy ward, caring for up to 60 patients per week. Working under the Enhanced Recovery Pathway, patients are discharged on day of surgery or the following day. We work closely with the Enhanced Recovery team including consultants and E/R specialist sisters.

Patient will be advised to attend Joint School prior to admission.

Main duties of the job

Under the direction of the Ward Manager, the post holder will be responsible for assisting the ward team, ensuring the smooth functioning of the unit through effective communication, general administration and appropriate interactions with patients, visitors and the multidisciplinary team.

Detailed job description and main responsibilities

Verbally welcome new patients onto the ward, admitting patients and checking details are correct electronically on the Trust PAS (Patient Administration System).

Use the Trust computer systems i.e. PAS (Patient Administration System), EPR, (Electronic Patient Record) E-mail, Microsoft Word and Excel. Registration of patients, inputting and collecting data, distributing this information to relevant personnel and acting upon the information received.

Retrieve patient information such as notes, X-rays scan reports etc. from within the Trust and external agencies. Notes and laboratory reports will be filed in accordance to CNST (Clinical Negligence Scheme for Trusts) standards. Preparing sets of notes for the Multidisciplinary team, for admissions, ward rounds and outpatients.
Ensuring appropriate levels of documentation are available i.e. addressograph labels. On discharge check all notes and charts are filed correctly in patient records, ensuring discharge summaries are complete and notes are sent to appropriate destination.

This may involve walking long distances on a regular basis between departments and a degree of physical effort is required. Manual handling training is mandatory on an annual basis.

Verbally, telephone and electronically communicate with estates department for maintenance repairs within department.

Organising meals for patients and canceling as appropriate, communicating with kitchen staff/dietician to ensure dietary requirements are satisfied.

In accordance with hospital Trust Guidelines, order and confirm patient transport at the request of the nurse in charge, prior to a patient’s discharge or other hospital appointment – complete transport and ambulance forms

Sort and distribute mail and re-address if necessary.

To be accountable for own practice, and take every reasonable opportunity to sustain and improve knowledge and professional competence.

Attend annual mandatory/statutory training to ensure adherence to Trust policies and current legislation.

Attend relevant in house training sessions that are pertinent to own clinical practice, in order to enhance knowledge and skills.

To assist in the creation of a good working atmosphere.

Assist team members in the maintenance and control of stock/equipment, which may include ordering, collecting, and delivering items between departments.

Order stationery and equipment on the ORACLE and inform the Ward Manager / nurse in charge when ready for authorization and chase up as necessary.

Maintains a working environment that enhances and promotes the health and safety of patients and staff and all visitors to the area.

Assist in the data collection for audit purposes, categorise if necessary and send to appropriate destination either verbally, electronically or by telephone.

Ensure adherence to Trust policies, procedures, guidelines and current legislation.

Maintaining good interpersonal relationships with clients and relatives whilst maintaining confidentiality/sensitivity, working closely with all members of the multidisciplinary team.

Communicating with external agencies.

Answering telephone and helping with any queries regarding patients and relaying messages to ward staff, doctors and patients. Dealing with calls from patients, relatives who may be aggressive or distressed and refer as appropriate.

Communicate/interact between departments within the Trust and external organisations, to arrange times for patients to undertake investigations and keeping the senior nurse informed of progress at all times.

Preparing post operative and discharge packs, to support the nursing team.
