As a Healthcare Support Worker in the Community you will be expected to provide support to our patients in a wide range of clinical community environments across the Crediton Community Nursing areas as part of Crediton, Moretonhampstead and Okehampton cluster in comfort of their own homes.
Set in vibrant market town, within the Dartmoor National Park you will also form part of the Moretonhampstead Community Nursing team within the Cluster, and we have a supportive and dynamic management team who truly value staff wellbeing and development.

Along with supporting clinicians with their patient workloads delivering clinical care delegated by a registered professional and contributing to the delivery of patient focussed care. The post holder will be part of an integrated health and social care team consisting of GPs, Nursing staff, OTs, Physios, Urgent Community Response Support workers and Social care team, who are responsible for the planning, implementation and evaluation of the patients needs and the delivery of high quality care.

You will be supported in your role by a Nurse Specialist (Community) and the operational management team. You should have an NVQ 3 in Healthcare or working towards a qualification or equivalent relevant qualification. We are looking for a Healthcare Support Worker who has a current driving licence and attributes to form part of a dynamic team to deliver innovative care to our patients.
