The Home First Team (South) are offering an exciting opportunity for a NMC Registered Band 5 Nurse to join the Community based Discharge to Assess (D2A) Service working in and around Stafford region.
The Team covers the whole of South Staffordshire and provides clinical support for patients in the community to support people to return home through the discharge to assess pathway following a period of acute care in hospital.
The team also work to facilitate safe hospital discharges for patients leaving acute care and to offer them nursing support in their own home environment.
The Home First Team prides themselves on the delivery of quality care to community patients, many with complex needs. All staff promote the philosophy of self-care and the promotion of independence whilst offering the facilities of rehabilitation and assessment.The ethos of the team is to support people to remain in their own home and to avoid unnecessary hospital admissions.
You will be responsible for will assessing, managing, planning and delivering care, to patients allocated to the service. You work with and support peers, Senior Nurse Co-coordinator (Band 6) and be a proactive and effective team member.
The service operates a 24/7 service, and you will be expected to work 5 shifts in 7 days on a rota basis. We are currently offering part-time and full-time opportunities.
Due to the nature of this role, you will be required to travel independently over a large geographical area.
Main duties of the job
As a Band 5 Nurse in the Home First Team, you will be able to initiate referrals to other health professional specialist services and agencies. To establish and monitor good working relationships with other health and social care professionals, statutory and voluntary organisations.
As part of this role, you will be expected to:
- Provide patients and relatives with information and education thus ensuring they have meaningful choices that promote dignity, independence and quality of life.
- Act as a role model and motivate team members in working towards innovative and high standard of practice.
- Participate and support the Senior Nurse Coordinator and Clinical Lead in the development of an effective team and the development of productive working relationships throughout the Trust.
- Support the Senior Nurse Coordinator and Clinical Lead in the development of a positive and ‘supportive’ team culture.
- Provide holistic assessments, planning, implementation and evaluation of care as part of the D2A process.
- Providing public health, health promotional programs to improve.
- Be responsible, and accountable, for service delivery to clients/patients.
- Improvement in health and wellbeing including mental health.
- Early supported discharge.
- Advise Senior Nurse Coordinator and Clinical Lead of any issues that may affect the patient or the service.

Qualifications
Essential criteria- Degree in nursing or equivalent
- Registration with NMC

Training
Essential criteria- Evidence of / willing to undertake post-registration training relevant to post
Work Experience
Essential criteria- Relevant experience / placement to the role including assessment and management of long term conditions, post-operative conditions and wound care
- Ability to manage time and caseload effectively.
- Supporting service users, families and carers in dealing with people with nursing needs
- Experience in the delivery of healthcare
- relevant placemements including surgical and medical
knowledge
Essential criteria- Evidence of clinical knowledge relevant to the post, with knowledge of care planning
- Understanding of Information Governance, Data Protection
- NMC Standards and Safeguarding Vulnerable Adults
- Awareness of standardised assessments and outcome measures
- Awareness of relevant NSF's and NICE guidance
Skills and Competencies
