Marie Curie, in partnership with University Hospitals Plymouth, is introducing a new service model aimed at enhancing end-of-life care, with a special focus on identifying patients who are approaching the end of their lives. This innovative service focuses on four key areas: Delivering hands-on care and emotional support to patients and their families in the Emergency Department.
Proactively identifying and assisting those nearing the end of life, ensuring timely transfers to their preferred place of care. Offering specialised care within a community hospital setting. Engaging a dedicated team of volunteers to provide companionship and practical support.
In joining Marie Curie as a Hub Coordinator, you will support the end-to-end care coordination for new and existing patients, including preparation, communication to families and input of care details to the internal systems. You will be required to undertake general administration of the computer system, to ensure that accurate computer records are maintained for referrals, nurse allocation, cancellations, packages of care and financial data.

This will also include the discussing and agreeing the allocation of Marie Curie Nurses to patients with the Senior Coordinator, managing the daily and monthly rotas of the Marie Curie nurses and making sure that the systems are up to date. With the nature of this role, you will assume responsibility for identifying and highlighting rapid discharge and emergency referrals, which may result in interruptions to other planned tasks which will require the ability to manage and re-prioritise your workload at short notice.

What were looking for: NVQ Level 3, or equivalent qualification (desirable) Excellent communication, inter-personal and organisational skills including professional telephone manner. Experience in a customer care or mediation role, including dealing with complaints in a health setting Ability to attend and contribute to planned team meetings, provide statistical reports and manage rotas.

Self-motivated to ensure workload is properly managed, working within specified timeframes to ensure targets are met.
