Advanced Pharmacist (Medicines Effectiveness)
As a specialist Medicines Effectiveness pharmacist, provide information and support the Lead Pharmacy Technician (Medicines Effectiveness) in managing the introduction of new formulary medicines to the Trust (including medicines with positive NICE TAs and PBR-excluded medicines).
This may include providing information and advice to facilitate creation of new drug profiles, electronic prescription entries, and any additional materials necessary to facilitate introduction of new formulary medicines to the Trust. As a specialist Medicines Effectiveness pharmacist, provide information and support the Lead Pharmacy Technician (Medicines Effectiveness) in publicising key formulary changes, including changes to BSW prescribing guidance / treatment pathways, to healthcare professionals across the Trust.
As a specialist Medicines Effectiveness pharmacist, provide information and support the Lead Pharmacy Technician (Medicines Effectiveness) inidentifying themes from audits of non-formulary and unlicensed prescribing, and in designing and implementing mitigations and learning interventions to improve formulary adherence and unlicensed prescribing.To provide internal and external education and training, to pharmacy staff and other healthcare professionals, in relation to formulary, commissioning and interface issues and evaluate training provided. To practiceas an advanced Pharmacy practitioner with special interest in at least one clinical specialty.
To provide a specialist clinical pharmacy service to patients in specified wards and clinical areas in line with clinical pharmacy standards and objectives. 8.
