An exciting opportunity has arisen for an experienced Senior Therapy Assistant with knowledge, skills and experience in intermediate care, rehabilitation, Rapid Response and admission avoidance.

We are looking for an experienced and well-organised individual with a passion for promoting independence and providing therapy intervention in a person's own home, utilising a home-first approach.

We are excited to welcome you to join our friendly team, where you will have the opportunity to work flexibly across the tier of services, which includes:

  • Bury Rapid Community Response Service
  • Killelea House (Enhanced and residential IMC)
  • Home-based Intermediate Care and Reablement

You will be part of a large multidisciplinary team that includes Physiotherapists, Occupational Therapists, Advanced Practitioners, Nurses, Social Workers, Care Support Workers, and Administrators.

The Service operates a 7-day service, currently covering the hours of 8 am to 8 pm, including weekends; therefore, the ability to work shifts to cover these hours is a requirement of the role.

The Intermediate Tier prides itself on providing strengths-based assessments and interventions to the people of Bury, helping them regain or attain as much independence as possible in their homes.

Our patients are at the heart of everything we do! We are looking for someone committed to being part of a healthcare team that delivers excellent care and support to our patients. To help us achieve this, you will be responsible for providing support work to aid the rehabilitation of patients.

You will carry your own caseload, perform patient therapy assessments, develop, implement, and evaluate individualised holistic treatment programmes, and deliver a high standard of evidence-based care.

Within all our Intermediate Tier services, you will work within and have support from a multi-disciplinary team, follow appropriate pathways of care, and liaise with and refer to other agencies as appropriate.

We seek someone hardworking and motivated with clinical, communication, organisational and time management skills. A flexible approach to day-to-day tasks is a must to meet the demands of the service. You will also need to travel across the service footprint, when necessary, in line with service needs, so access to transport is essential.
