Using the model of early identification, preventative interventions and enablement ethos you will support people by implementing person-centred goals that empowers them to access opportunities to optimise their occupational performance, quality of life and their physical, social and psychological wellbeing which in turn will avoid deteriorations, hospital admission or emergency department presentation.
The successful candidate will need to have the analytical and critical thinking skills to gather and provide ongoing evaluation, evidence and vision for future practice development. The Post holder be supported by and will work under the supervision of the Professional Lead Occupational Therapist (OT) for the Borough.
