If you are a RMN with experience working with people suffering severe mental illness whether this be inpatient, or community based we would love to hear from you.
Of the current posts available, one is full time (37.5 hrs) and the remainder is for 35 hrs. These posts are being extended to allow for applications from newly qualified nurses, or those practitioners currently working at a Band 5 and looking to develop with support towards a Band 6 whilst working in the community.
The Service is a specialist team providing assessment, treatment and support for individuals suffering from organic and functional mental health illness. Experience of working with older adults with organic/functional mental illness in an inpatient or community setting would be an advantage.The ideal candidate will be enthusiastic, self-motivated and hard-working, with a positive approach to delivering high quality care to patients and be willing to work collaboratively with carers and other professionals. You will carry a defined caseload and manage this with the support of the team leader and the wider team.
We work with a multitude of services across the Christchurch and surrounding areas to ensure the mental health needs of the older adult population is met.
We are a busy CMHT and we are looking for someone who is reliable, committed and motivated with excellent communication and organisational skills and the ability to prioritise their caseload whilst balancing service demands. In return we are ready to offer you comprehensive and structured management supervision, management you can approach any time and peer support.
The post holders will work as part of an established integrated multi-disciplinary team. This team is made up of a Psychiatrists, Social Workers, Occupational Therapists, Clinical Psychologists, Nursing staff, Support Time Recovery Workers and Admin & Clerical staff.
The primary role responsibility is to act as dedicated Care Coordinator for a caseload of patients with whom you would provide person centred, holistic care to assist in aiding their wellbeing and recovery.
