We have an exciting opening in our team to cover Maternity leave for 12 months on a fixed term or secondment . This is a great opportunity for an experienced Children’s Nurse to join our dynamic and friendly Children’s Community Complex Care Team.

As the team leader you will be responsible for managing a team of nurses and support workers. You will also be responsible for overseeing the completion of the continuing care assessments and the provision of care packages.

We are looking for a motivated individual with excellent communication skills.

You will act as a role model, demonstrating high standards of care and provide clinical leadership. You will be responsible for assessing, planning, implementing and evaluating children’s nursing care in the community setting. Knowledge of the children’s continuing care framework and partnership working with the commissioners and the multi-disciplinary teams are vital to this role.

You will need to be able to work flexibly. Although the role is predominantly office hours, Monday to Friday, the service is 24 hours and you will on occasions be required to work nights and weekends to cover shifts and supervise Band 3 support workers.

You will also be required to deputise for the Head of Service / Matron in their absence.

  • To have day-to-day leadership, management and development responsibility of a dedicated children’s service and to hold a designated paediatric caseload.
  • To work as an autonomous clinician in a defined area to develop and provide expert clinical advice, support and care to a defined group of patients.
  • To oversee the completion of the children’s continuing care assessments, work in collaboration with the commissioners to offer appropriate care provision.
  • Monitor and review the implementation of treatment plans and make changes within agreed protocols, pathways or models for clinical effectiveness.
  • Empower children and young people and their families/ carers through appropriate support and education.
  • To establish and maintain individual and team communication networks within primary, secondary and tertiary care settings to ensure multi-disciplinary and multi-agency training programmes.
  • Develop working relationships with primary, secondary and tertiary clinicians to ensure patient care is of a high quality and appropriately delivered and be a source of clinical expertise for other practitioners
  • Develop protocols and pathways of care based on current evidence based guidelines to ensure best practice.
  • Provide clinical supervision and leadership, to ensure high clinical standards, and to facilitate the development of individual team members.
  • To work with the Children’s Community Nursing Manager in developing services particular to children’s complex care.
