Rehabilitation Assistant - Tewkesbury

o Monitoring progress and function of people within their reablement programme and contributing to the ongoing assessment of their rehabilitation needs by liaising with team colleagues and recording progress, in compliance with organisation policies and procedures o Identifying when a safeguarding alert may need to be raised, ensuring the timely escalation of suspected abuse and self-neglect promptly o Supporting people and where appropriate their families and carers, by advising them on all aspects of their Reablement plan including the correct use of equipment o Adhering to all health and safety instructions.

Report to line manager any incidents, emergencies or accidents. o Connect people to community resources, promoting digital inclusion and the use of assisted technology e.g. telecare o Be an active participant of your team, maintaining your own continuing professional development, to ensure you can deliver the best quality care and support to the people of Gloucestershire.
