Staff Nurse - Band 5 - 30 hours per week

Identify and assess physical, psychosocial and spiritual needs of patients and their families. To support Sister/Nurse in Charge and Senior Staff Nurses. To assist in developing an environment in keeping with values and behaviours of the Hospice.
Demonstrate the ability to develop a greater awareness and understanding of the therapeutic options available to meet the holistic needs of the patients requiring specialist palliative care. Develop a positive, realistic and supportive attitude to ensure the needs of the patient and their families/carers are met.
Demonstrate an ability to draw information from the nursing/medical history assessments in order to plan and implement the patients individual care, by setting attainable goals and objectives to help the patient attain these goals. Document and evaluate given care and discuss progress, condition and treatment with members of the multidisciplinary team, alerting the Sister/Senior Nurse of patients changing health needs.
Assist with teaching patients and their families/carers in preparation for changes in treatment, expectations and discharge for home care. Awareness of potential side-effects of any treatment in progress, adopting a problem-solving approach to minimise potentially adverse reactions.
Give information, support and advice to families and friends following the death of a patient, referring them to other services/agencies for support if necessary. Communicate effectively with members of the multidisciplinary team, patients and their families.
Awareness of all Hospice policies and procedures and collaborates with other staff to ensure that these are complied with. Develop time-management skills and the ability to prioritise nursing care for a group of patients. Assist more senior members of the nursing staff in the organisation and management of the clinical area ensuring that the principles of individualised care and nursing are maintained.
Develop an understanding of nursing team management with the guidance of the Sister/Senior Nurse. Take responsibility for ensuring all mandatory training is up to date. Ensure the appropriate delegation of the care of patients and other duties when co-ordinating the ward team.
Promote a safe environment for all patients, visitors and staff. Report incidents, accidents and complaints to the Sister/Senior Nurse, completing the relevant documentation. Undertake other relevant duties as required by the Sister/Senior Nurse.
Ensure that equipment is in good working order and report any defects to the Sister/Senior Nurse. Assist in the orientation, guidance and teaching of new staff if delegated as mentor by the Ward Sister. Assist in the monitoring of stock levels and ordering of medication each week.
Confidentiality: To be aware of the confidential nature of the work undertaken in this role, and of the Hospice as a whole. Any matter of confidential nature relating to patients, carers, relatives, staff or volunteers must not be divulged to any unauthorised person.
Data Protection: To be aware of the requirements of the Data Protection Act and follow local codes of practice to ensure appropriate action to safeguard confidential information. Health and Safety: To be aware of the policies and practices in relation to Health and Safety and to support the implementation and monitoring of compliance with policy. Further Training: To liaise with external individuals, advisors and organisation and, if required, attending training to develop the skills and knowledge of the post holder for the benefit of the Hospice.

Education & Professional Development: Ensure own registration with the Nursing and Midwifery Council (NMC), and is aware of the NMC Code of Professional Conduct and Scope of Professional Practice. Take responsibility for individual professional development by keeping up to date with current advances in nursing practice and research particularly in the field of palliative care, for example by attending relevant study days and courses and taking responsibility for updating own professional portfolio, in accordance with the NMC, adhering to the Hospice study leave policy.

Positively commit to staff appraisal, performance and clinical supervision to ensure personal professional development. Assist in orientation, guidance and teaching of new staff. Support and advise other members of the team in order to promote and maintain staff morale.
Participate in educational programmes offered to staff and specifically attend study days identified to meet staff nurse development aims and objectives. Quality & Service Development: In conjunction with senior management and colleagues within the multi-professional team to respond positively to identified change initiatives.

Engage in research, audit and quality initiatives. Introduce research-based findings into clinical practice and show commitment to research by initiating or participating in research projects where possible. With other clinical staff members evaluate the effectiveness of the service and identify areas for improvement.
