Occupational Therapist - Community Learning Disability Team

Hold a caseload; this will include those people with complex Occupational Therapy needs. For people on their caseload, to be responsible for the whole process of assessment, care planning, monitoring and review. Act as an experienced member of the CLDT in relation to risk assessment giving advice and direction as required.
Contribute to Continuing Healthcare Assessments. Monitor, evaluate and modify interventions in order to measure progress and ensure effectiveness. Provide education, consultation and advice to care settings and day activity placements. Have knowledge of legislation pertaining to Learning Disabilities eg mental capacity Act, Best Interest, Valuing People, death by indifference.
Carry out complex risk assessment involving wide consultation and collaboration. Able to carry out a moderate physical effort throughout the working day. Be required to work with people who have needs around challenging behaviour Be required to carry out complex assessments and interventions in difficult circumstances, e.g., poor environmental conditions, or at times when service users' behaviour is unpredictable.
