Graduate Mental Health Worker | Central and North West London NHS Foundation Trust
An exciting opportunity has arisen for a Band 4 Graduate Mental Health Worker in South Westminster Adult Mental Healthhub. This is a valued post in our multi-disciplinary team, working three days with our psychology service and two days in our Triage service.
We are seeking applications from a Mental Health graduate or from existing mental health workers with significant years of experience working within a community mental health setting.
We will accept applications from those with a desire to work within our integrated model of service delivery. The position is based at our South Westminster Mental Health hub 190 Vauxhall Bridge Road SW1V 1DX.

You will be an excellent communicator and have good organisational and IT skills.
As part of the Triage service, you will join the triage team in screening and processing new mental health referrals into the service. The post holder will also take a lead in supporting the Therapies meeting where onward referrals are then discussed amongst therapies and triage colleagues.
You will join our Psychology service and will take a lead in implementing performance indicators, such as 45-day check-in phone calls and co-constructing Dialog Plus care plans with clients waiting for psychological therapy.
You will also carry a caseload under weekly supervision with a qualified Psychologist, providing individual psychological interventions and collaborate in our Psychosocial group program, where appropriate, which delivers walking groups, Grounding groups and mindfulness groups.

- Participate in the provision of the initial screening, triage and care navigation service of the mental health hub.
- Processing and screening referrals received into mental health hub – shared responsibility with Pathway Coordinator
- Developing links with local community services that will contribute to the repertoire of resources available to mental health users in Westminster to enable recovery and where appropriate, signposting service users to local services for support
- Participate in triage/assessment of new patients who have therapy needs
- Supporting weekly Hub therapies meeting – prepping cases for discussion, taking minutes and following up on actions
- Take an active role in supporting the team in the provision of high quality individualised, evidence-based care under the supervision of clinical staff
- Running groups with service users with support from psychologists/Occupational Therapists in the team e.g. Grounding, Sleep Management, Skills, Relaxation etc
- Support senior staff in carrying out day to day tasks associated with provision of care such as written and verbal liaison with service users, carers and other providers of care.
- Carrying out check in/welfare calls with service users who are awaiting therapies treatments in the hub
- Communicate effectively with service users and their families/carers with written care plans or signposting information
- To provide active mental health monitoring of service users and providing feedback on changes in presentation ensuring that marked changes are shared with appropriate professionals in a timely fashion
- Ensuring that professional communications are conducted at the highest standards being effective and efficient.
- Ensuring all relevant information regarding the service user is documented clearly and in a timey manner on SystmOne to provide an accurate audit trail of care and decisions
