As an Experienced Social Worker in the Assessment Team, you will play a vital role in safeguarding and promoting the welfare of children and young people in Medway. You will conduct comprehensive assessments, develop care plans, and coordinate appropriate services to ensure the best possible outcomes for vulnerable individuals and families.

Key Responsibilities:

  • Conduct thorough assessments of children and young people, identifying their needs, risks, and protective factors.
  • Develop and implement care plans, working in partnership with families, professionals, and support networks.
  • Monitor and review the progress and effectiveness of interventions, making adjustments as necessary.
  • Provide expert advice and guidance to colleagues, ensuring the delivery of high-quality social work practice.
  • Collaborate with partner agencies, such as health services, education, and the police, to ensure a coordinated approach to service provision.
  • Maintain accurate and up-to-date records, adhering to relevant legislation, policies, and procedures.
  • Engage in continuous professional development, keeping abreast of the latest research and best practice guidelines.

Requirements:

  • Qualified Social Worker with current registration and a recognized social work qualification.
  • Substantial experience in a social work role, preferably within an Assessment Team.
  • Sound knowledge of relevant legislation, policies, and procedures.
  • Strong assessment and decision-making skills, with a focus on evidence-based practice.
  • Excellent communication and interpersonal skills, enabling effective engagement with children, young people, families, and professionals.
  • Ability to work autonomously and as part of a team, demonstrating resilience and adaptability in a challenging environment.
