We are looking for a highly motivated Diabetes Nurse Specialist, who is passionate about diabetes ready to make a major contribution to the care of people in the community with diabetes.

The post holder will be responsible, as part of a multidisciplinary clinical team, for supporting the care of people with diabetes at their GP surgery and in secondary care (4 days in the GP surgeries and 1 day at the Homerton Hospital outpatient setting).

The post holder will ensure that people receive the necessary care and treatment to best manage their long-term condition. This will be achieved through the application of specialist knowledge, skills and practice.

Main duties of the job

You will be a registered nurse with a post registration nursing course in Diabetes with experience as a Diabetes Nurse Specialist.

You should have proven management and leadership skills as well as the ability to demonstrate initiative and innovation, effective communication skills and the ability to organise your own caseload.

You will also be able to demonstrate effective teaching and presentation skills as you will be responsible for providing education and support to primary care teams. You will also be required to provide clinical care to a caseload of patients with diabetes.

You will be able to work across primary care providers in City & Hackney.

  • To support and review patients to ensure that the best management of their diabetes.
  • The post holder is responsible to manage a caseload of patients across a number of practices in City & Hackney involving both face-to-face and remote working.
  • The post holder works as an autonomous practitioner delivering knowledge and expertise within the specialist area.
  • To have good communication skills and work within the specialty team to ensure cohesive multidisciplinary management of patients.

Education / Qualifications

Essential criteria
  • RGN Level 1
  • Current registration with the NMC on relevant part of the register
  • Relevant post basic Diabetes course OR evidence of on going relevant postgraduate courses associated with speciality
Desirable criteria
  • Diploma in Diabetes
  • Working towards a Masters Degree
  • Independent prescribing

Skills/ Abilities

Essential criteria
  • Excellent verbal and written communication skills
  • Excellent clinical skills within the field of Diabetes nursing
  • Excellent time management and organisational skills
  • Able to work independently and as part of the MDT
  • To have established leadership skills
  • Able to make decisions and problem solve
  • Able to work under pressure and prioritise
  • Able to use technology (Microsoft Office, Microsoft Teams, Libre view, Dexcom Clarity, Glooko)
Desirable criteria
  • Research/audit, data collection and input skills

Experience

Essential criteria
  • Substantive post registration experience
  • Experience in educating families and NOKs
  • Experience in teaching to HCPs (training staff)
  • Experience in mentoring junior members of the team
Desirable criteria
  • Independent Prescribing

Knowledge

Essential criteria
  • Expert level of knowledge of diabetes nursing
  • Ability to act as a clinical expert and resource for all staff in the multi-disciplinary team
  • Understanding the needs/demands of nursing in a multi-cultural environment
  • Knowledge of NICE guidelines and best evidence in diabetes
