As an Assessment and Move on Worker, you will play a pivotal role in supporting individuals who are navigating their transition from various support systems into sustainable independent living. Your primary focus will be on assessing the needs of individuals, creating tailored move-on plans, and providing essential guidance and support throughout the process.
This position requires a compassionate and proactive individual who can work collaboratively with both clients and a multidisciplinary team. You will be responsible for developing strong rapport with clients, helping them identify their personal goals, and aligning available resources to meet these objectives.
The job involves a mixture of administrative duties and direct client engagement, ensuring that each individual's unique circumstances are thoroughly understood and addressed. Your expertise will contribute to empowering individuals with the skills and confidence needed to successfully make the move toward independence.

Responsibilities
Conduct comprehensive assessments to identify the needs and preferences of individuals.
Develop personalized move-on plans in collaboration with clients, focusing on their goals.
Provide ongoing support and guidance through the transition process.
Coordinate and collaborate with external agencies and service providers to facilitate support.
Monitor and evaluate the progress of clients against their move-on plans, making adjustments as needed.
Maintain accurate records and documentation of client interactions and progress.

Advocate for clients to ensure they receive appropriate services and resources.

Requirements
A degree in social work, psychology, or a related field is preferred.
Experience in supporting individuals in transition or similar social services.
Strong interpersonal and communication skills for effective client engagement.
Ability to conduct assessments and create action plans tailored to individual needs.
Familiarity with relevant legislation and policies in social work.
Strong organizational skills and attention to detail in documentation and reporting.

Empathy, patience, and a genuine desire to help individuals achieve independence.

Hours Per Week: 36.00

Pay Per Hour: £19.92
