The Community Long Term Team ( CLTT) in Peterborough City Council (PCC) work closely with Cambridgeshire Learning Disability Partnership (LDP) to provide an integrated health and social care service for adults with Learning Disabilities living in Cambridgeshire and Peterborough.

The CLTT aim is to ensure the assessment and provision of person-centred specialist health and social care services that promote an individual’s right to choice, independence, health and wellbeing, in line with their assessed needs and enables them to reach their full potential.

We have an exciting opportunity to recruit an enthusiastic Band 5 Community Learning Disability Nurses to work across the Peterborough area. You will have experience relevant to working within a community-based role, supporting people with learning disabilities.

As an autonomous practitioner you will manage a caseload providing assessment and interventions in relation to the health needs of adults with a learning disability.

The delivery of specialist nursing within Peterborough , working alongside our colleagues within LDP of Cambridgeshire
  • To participate in the delivery of a countywide specialist Learning Disability nursing service under the guidance of a senior Nurse.
  • With support from a senior Nurse work with identified service users providing assessment, identification and treatment of the health needs of adults with a learning disability.

  • The post holder will demonstrate knowledge and understanding of working with Adults with a Learning Disability and be willing to undertake appropriate post registration training.
  • To have an understanding of Health and Social care policy and legislation for the service user group in relation to your professional practice.
  • With the support of the senior nurse prioritise and maintain a personal caseload within the context of the Community Long Term Team and in conjunction with our Colleague’s within the LDP .
  • To participate in the assessment, identification, treatment and management of the specialist health and nursing needs of the learning disability population. This includes risk assessment, assessment of complex conditions e.g. challenging behaviour including forensic issues, palliative care, mental health, epilepsy etc.
  • To provide advice and support to service users, carers and partner agencies in regard to people with learning disability.
  • To liaise and communicate effectively and sensitively with other professionals in the MDT, the CLTT, the LDP and external agencies (e.g. Older peoples teams, Mental Health Teams, Drug and Alcohol, Palliative and Children’s Service’s) providing specialist LD advice as necessary.
  • To provide interventions in relation to the implementation and evaluation of specialist programmes of care which support the health care of individuals or groups with Learning disabilities and complex needs.
  • To have a good understanding of the specific needs of the service user group including at times of crisis. e.g. during placement breakdowns, mental health and physical health relapses and periods of challenging behaviours
  • To recognise the impact of a person’s communication needs and tailor interaction to meet this need.
  • To have knowledge of multidisciplinary working with parents with learning disability.
  • To work in service users homes and a variety of community settings where conditions may pose a risk, including lone working.
  • Under the supervision of the senior nurse write timely professional reports/ records and action plans in line with professional guidelines that comply with CPFT and County Council record keeping policies as appropriate.
  • To provide nursing advice in relation to individual case work to professionals within the CLTT
  • To work to reduce health inequalities for people with a Learning Disability by working with partners in primary and secondary care.
  • To have an understanding of local health needs and as part of the CLTT nursing service respond to changes and care provision as outlined in government initiatives.
