Are you an experienced Registered Nurse seeking an opportunity to work with the Amber Valley Rapid Response Nursing Team?

An opportunity has arisen for a highly motivated nurse to join us , this is an excellent opportunity for you to use your clinical and leadership skills to support the team.
The service provides acute (short-term) care in patients' own homes and Residential Care Home settings. The team promote a self management approach to patients with their health conditions.
A key priority is for rapid nursing staff to educate patients on their health needs and to provide a self management approach to patients with their condition.
The service provides cover 7 day per week, 08:00–18:30 and incorporates a rapid response service supporting patients with urgent nursing care needs.
You will have the opportunity to develop your skills of specialist nursing assessment and treatment in a community setting, alongside developing additional generic skills to provide a comprehensive assessment to a variety of complex patients.
You will be well supported in the role by a skilled multi-disciplinary team.
Main duties of the job
Some of your responsibilities would include:
Urgent (same day) community nursing referrals
Co-ordinating the nursing response
Responding to patients in immediate need
Clinical triage of referrals
Managing the caseload
Leadership of staff
Developing service and supporting your colleagues

Education, Qualifications and Training
Essential criteria- Registered nurse with current NMC registration
- Relevant nursing degree or equivalent relevant experience
- Student mentorship
- Have or be required to undertake a leadership development programme
- Recognised teaching qualification
- Leg ulcer management
- Palliative care
- Nurse prescribing qualification
- Tissue viability
Experience and Knowledge
Essential criteria- Post registration experience
- Case management/ ward management experience
- Long Term Conditions
- Community Experience
- Proven clinical expertise, knowledge and experience of implementing service improvement
Skills & Attributes
Essential criteria- IT and administrative skills
- Can demonstrate knowledge of research based practice and quality
- Excellent communication skills both verbal and written

Essential criteria- Access to transport is essential for this post, as you will be expected to travel around various areas of Derbyshire County in order to carry out required duties
