RESPONSIBLE FOR:Management and on-going development of the Hospice at HomeTeam including Senior Staff Nurses, Healthcare Assistants and supporting students on placement. OVERALL PURPOSE: To ensure high quality care to people in their own homes and management, clinical leadership and supervision to the Hospice at Home Team.
To be responsible for managing the Hospice at Home caseload. Manage the resources associated with the team. Develop the service in line with the organisation business plan. Continuously monitor the quality of care provided using appropriate metrics.
Contribute to the Multidisciplinary Team approach to patient care. Facilitate staff engagement. Innovate practice and keep abreast of evidence base practice, applying this to our cohort of patients. Manage individual staff performance and learning needs.
JOB PURPOSE: Provide inspirational leadership to support and fulfil the objectives of the service. Innovate practice in line with evidence. During clinical hours to provide support to patients in their own homes who have palliative and end of life care needs, raising clinical standards and promoting a learning environment for other health care providers.
To be responsible for planning, assessing, implementing and evaluating care in partnership with the District Nursing Team and out of hours providers. To work a minimum of 10 non-clinical hours per week outside of normal working hours. CLINICAL RESPONSIBILITIES To provide palliative care advice and support to patients, their families and carers.
To plan, assess and implement patient care plans in conjunction with the Primary Health Care Team. Monitor patients condition and report significant changes to the Community Palliative Care Team via morning handover, GP and District Nursing Team incorporating high standards of individualised patient care.
Liaise effectively with other health care professionals in the community and within the Hospice, receiving and sharing information through effective communication. Be responsible for managing the process of receiving and responding to referrals to the Trinity Hospice at Home Service.
Prioritising workload of the Team and managing resource allocation as required. Be prepared to cope with incidents or emergency situations as they arise and report them to the Senior Manager on duty. To contribute to the District Nurses assessments of patients as well as the development and implementation of programmes of care for patients in their own homes.
To carry out specific treatments and clinical procedures, basing clinical practice on current research based evidence. To administer drugs and assist with their ordering in accordance with NMC standards for the Administration of Medicines. Ensure that relevant health and safety procedures are maintained within patient areas, either at their home or within the Hospice.
To use advanced communication skills to support carers and families of patients receiving services from Trinity Hospice at Home Service. To verify when a death has occurred, notifying families and carers as appropriate following the procedures carried out within the community.
To ensure that patients and carers are identified and appropriately referred to other services. Be responsible for co-ordinating flexible care provision to enable patients to remain at home. Maintain accurate patient records in line with Hospice guidelines.
PROFESSIONAL/MANAGERIAL RESPONSIBILITIES Be accountable to the Community services manager and/or Head of Community Services for the overall performance of the team. Ensure Confidentiality from staff at all times and ensure safe keeping and use of patient data.
Ensure staff are maintaining all required mandatory training needs and working within policies and procedures for the service. Support the development of nursing staff on a daily basis including induction, and completion of clinical competencies.
Undertake performance review and manage poor performance as appropriate with/supported by the Community Teams Manager. Manage sickness, absence and rotas in line with policies. Undertake PDRs and 1-1s with staff supporting their development and achievement of objectives.
Support the recruitment process. GENERAL RESPONSIBILITIES Maintain professional registration in line with NMC requirements for revalidation. Be responsible for own learning needs. Act in accordance with Trinity Hospices value and behaviours. Be a role model to staff in relation to learning and a learning culture.
