Shift Pattern: Mon-Fri 7.5hrs per day alternating 8-4 and 8.30-4.30 weekly. (can be discussed)

- Manage caseload on an acute medical ward using a band 3 support worker (with band 7 support) including obtaining handover and prioritising patients by clinical need.
- Competent in making comprehensive patient assessments and support an early supported 'home first’ discharge approach.
- Able to make timely complex discharge plans in a busy hospital environment.
- Able to use electronic patient records.
To be successful in applying for this role you will need:
- Essential:
- HCPC registered
- Acute NHS hospital experience
- Complex discharge planning, early supported discharge
- Broad inpatient physiotherapy experience
- Relevant Qualifications
- Registration with the relevant Registration Body if applicable.
