Senior Urgent Response Practitioner - Home Based Treatment Team

Are you a Qualified Mental Health Nurse, Social Worker or Occupational Therapist with urgent care experience that is looking to progress and lead on the development of our Home-Based Treatment Team (HBTT)?
An exciting opportunity has arisen within Salford HBTT as a 12-month fixed term piloting a senior role in responding urgently to referrals from our Mental Health Liaison and Inpatient colleagues, offering alternative to inpatient admission for people with complex presentations.The post holder would lead on this initiative, seeing service users face to face but also supporting HBTT staff members in building their confidence in working with people with complex presentations. The post holder would attend board rounds and patient flow meetings to gain an understanding on where service pressures may be, and proactively support to reduce these within this role in the HBTT as well as proactively advocate for people to recover at home in the least restrictive way.
As a senior role, there will also be a requirement to supervise and support staff member within HBTT in their professional development.
Main duties of the job
The post holder will –- Focus on ensuring a rapid response to all urgent referrals coming into the service, alongside playing an integral role in supporting patient flow through the system. This will include:
- Supporting the service to develop and maintain robust gatekeeping processes, incorporating the principles of person-centred care and least restrictive interventions.
- Proactively identifying and facilitating supported early discharges from our inpatient settings.
- Supporting with admission avoidance from MHLS and community teams.
- Supporting the team development to meet the Core Fidelity Standards.

Qualifications
Essential criteria- Degree in RMN, Occupational Therapy or Social Work
- Additional qualification relevant to job role
Experience
Essential criteria- Acute / urgent care experience
- Experience within a leadership role previously
Skills
Essential criteria- Ability to manage complex cases autonomously
- Line management and staff development skills
Knowledge
Essential criteria- Understanding of the function and purpose of a Home Based Treatment Team
- Knowledge of the functions and purposes of our MHLT and inpatient colleagues
