We are pleased to offer this new and exciting opportunity to join the new Mid and South , Older Adult Admission Avoidance & Home Treatment Team. As part of the transformation agenda a new service is being formed to provide support to Older Adults, with Frailty and Functional Mental Health needs.On a Fixed term contract basis for 6 months covering 37.50 hours a week.

As part of this developing service you will help support older adults with functional mental health issues to meet increased needs / rising risk to support admission avoidance and provide home treatment with a Home First approach to support.You will work with a new and growing team, the existing community services and the established Dementia Admission avoidance services across Mid and South Essex
