Due to expansion within the Stockport Home Treatment Team an exciting opportunity for 2 full time Early Discharge Coordinators
Your main base will be at Stepping Hill Hospital.
This post is open to Nurses, Occupational Therapists or Social Workers.
The Stockport Home Treatment Team is a large multi-disciplinary team, consisting of a Consultant Psychiatrist and junior doctors, Team Manager, Deputy Manager, Psychologist, NMPs, Lead Nurses, Occupational Therapist, Social Workers, Support Workers, Pharmacy Technician and Administration staff.
We are in an exciting innovative position of piloting this new role within the team, which means enhancing our support offer on the acute wards, supporting early discharge to the Home Treatment Team.
The postholder will work closely with both the Home Treatment Team and the acute wards at Stockport, enabling a smooth discharge from hospital.
This well-established, multi-disciplinary team offers a high quality assessment and home treatment service where possible as an alternative to hospital admission, to individuals experiencing acute mental health crisis.
Main duties of the job
We are currently working to enhance our team in line with the Fidelity model for Crisis services in conjunction with all HTTs .
You will work within the service specifically as part of the MDT and have close links with the consultants on the wards and the Psychiatrist and Psychologist from the Home Treatment Team.
You will participate in Ward rounds for patients on the ward, coordinating patient discharge. Additionally, you will work with other colleagues across assessment services to enhance care provision.

You will be able to keep our commitments, demonstrate respect and dignity, work well under pressure and have very good communication skills, you will be working in a fast paced workplace where diversity is a focus.
You will have experience as a qualified practitioner within mental health and have strong leadership skills.

Detailed job description and main responsibilities
Main Duties and Responsibilities- Support the admission and delayed discharge activities within acute inpatient services. Attend weekly delayed discharge meetings, ensure s117 processes are followed, identify patients who requires accommodation in the community, make
- Minimise incidence of bed crises, using knowledge of impending discharges and extended periods of leave and placements breaking down in the community.
- Actively be involved in the delayed discharge process and assist the MDT to facilitate timely discharge and escalate cases as appropriate.
- Promote and foster supportive relationships with key stakeholders such as ICB, Accommodation Providers, Local Authority and Pennine Care
- Develop and maintain processes for effective discharge which may include accessing funding and placement for securing non-NHS beds.
- Proactive in ascertaining the Trust’s position regarding bed management resources.
- Liaison with appropriate teams and services in the management of interface issues.
- Ensure accurate and timely recording of information.
- Attend MDT meeting, risk panel meetings, complex case meeting and liaise with ICBs around placements and support the coordination of multi-disciplinary team reviews for complex delayed discharges.
- Maintain and demonstrate up to date knowledge, ensuring any gaps in clinical competency are addressed whilst also staying compliant with all business policies and procedures.
- Be a resource to junior members of staff, providing training, supervision and advice
- Ensure recommendations from national policy and guidance are communicated, understood and implemented.
- Support people on home leave from acute wards and whilst transitioning to the Home Treatment Team or borough crisis accommodation. Essential criteria- • RMN / Social Worker / Occupational Therapist
- Completion of Preparation for Mentorship / Practice Teacher / Fieldwork Educator qualification or willingness to complete
- Short courses and experience to post graduate diploma level
- Evidence of on-going CPD
- Post registration experience.
- Proven experience of working with clients with complex needs.
- Experience of working with individuals who present to mental health services in crisis and have symptoms of mental health illness
- Experience of working on acute psychiatric wards or Home Treatment Team
- Experience of working in multidisciplinary teams.
- Experience of providing supervision to junior staff
- Understanding of current mental health legislation, Safe Guarding and Child Protection Legislation
- Awareness of NICE Guidance in the treatment of mental health problems and knowledge of treatments and interventions appropriate for individuals with mental health needs Understanding of confidentiality and data protection issues.
- Knowledge of psychosocial assessment and risk assessment / management processes
- Comprehensive mental health and risk assessment skills.
- Ability to construct effective packages of care and risk management plans
