Family Support and Partnership Care Ltd is a team of compassionate Social Health Care Workers, Family Support Workers, and Social Workers who provide, 24/7 Short Breaks Services, Personalized Care, and Social Work Services to children and adults with complex health and social care needs.

We are currently looking for compassionate and dedicated Family Support Workers to join our team on a part-time basis. The role involves working with vulnerable clients; therefore, successful candidates will undergo enhanced DBS checks, and credible reference checks are also mandatory.

Responsibilities
  1. Assessment and Planning:
  • Ongoing assessments of families' needs and strengths.
  • Develop and implement individualised family support plans.
  1. Support and Advocacy:
  • Provide emotional and practical support to families.
  • Advocate on behalf of families to ensure they receive appropriate services.
  1. Resource Connection:
  • Identify and connect families with relevant community resources (e.g., housing, healthcare, education, financial assistance).
  • Maintain up-to-date knowledge of local resources and services.
  1. Education and Skill Development:
  • Provide information and education on parenting, child development, and healthy family dynamics.
  • Offer support around life skills, budgeting, and other relevant topics.
  1. Crisis Intervention:
  • Respond to family crises and provide immediate support.
  • Develop safety plans and connect families with emergency services when necessary.
  1. Documentation and Reporting:
  • Maintain accurate and detailed case notes and records.
  • Prepare reports on family progress and outcomes.
  • Ensure confidentiality and compliance with organisational and legal standards.
  1. Collaboration and Networking:
  • Work collaboratively with other professionals and agencies.
  • Participate in multi-agency meetings and case conferences.
  • Build and maintain strong working relationships with community partners.

The role involves working during the daytime and weekends and Out of out-of-hours calls.

Experience

Relevant qualifications or experience of working with vulnerable children or related experience. Ability to work independently and manage crises.
