Home First are looking for a Band 6 Occupational Therapist. This post will give great opportunity to gain varied experience in work across the Home 1stTherapy pathways. This is a role that can provide valuable community and inpatient experience to develop rehabilitation and supported discharge skills.

We provide therapy assessments and interventions for community admission avoidance and intermediate care needs, discharge support to the local acute hospital and inpatient rehabilitation to the Seacole Virtual Community Hospital.

Working closely with our wonderful team of Rehabilitation Assistants, Physiotherapists, Nurses and Pharmacy Technicians, this role will require you to provide comprehensive physical assessment, advice, treatment and discharge planning for the Home 1stcaseload.

To provide Occupational Therapy assessments and deliver person centred

rehabilitation and individualised goals.
  • To undertake all aspects of clinical duties as an autonomous practitioner.
  • Working in the Community pathways, assisting in admission avoidance assessments, the Getting People Home pathway to facilitate early supported discharge from the acute hospital and Inpatient rehabilitation pathways.
  • To communicate effectively and work collaboratively with all members of the multidisciplinary team and other services within Home 1st, as well as outside agencies.
  • Ability to work in capacity as a Trusted Assessor.
  • Participate in development of integrated pathways.

Here at Home First, we pride ourselves in the ongoing development of staff, we have successful applicants completing ACP and Apprenticeship courses for both Physiotherapy and Occupational Therapy and have regular students on placement with support from our Practice educators.

Working closely with our Professional Lead Therapists and Team Leads, the continuing development of the academic learning and vocational skills of the therapy team is imperative. Your participation in staff supervision, training and completion of competencies and observation of practice is key to the provision of the ongoing standard of care we expect at CNWL.

You will receive a comprehensive induction, followed by a tailored development plan, which includes self-directed learning and collaborative working within Home1st and its partners.

But it doesn't stop there .... we can offer coaching and opportunities for daily debriefs, access to remote working, regular 1:1 formal and informal supervision, peer group CPD sessions and in-service training, QI project work and volunteering for champion roles.

Home1st is based in Bletchley and the modern office is a central hub for multi-disciplinary working. You will be provided with ICT laptop, mobile phone, uniform and have mileage expenses paid for.
  • To be professional and legally accountable for all aspects of your work including the management of patients in your care.
  • To use specialist professional and clinical knowledge across a range of procedures based on a sound knowledge of evidence-based practice and treatment options, using clinical assessment, reasoning skills and knowledge of treatment skills.
  • To undertake assessment of patients, using clinical reasoning skills and manual assessment techniques in order to diagnose clinical need.
  • To assess patient understanding of treatment proposals, gain valid informed consent and have the capacity to work within a legal framework with patients who lack capacity to consent to treatment.
  • Ensure the participation of patient’s families and carers in decision making and planning mutually agreed objectives
  • Use a range of verbal or non-verbal communication tools to communicate effectively with patients, relative’s carers and other health and social care professionals to progress rehabilitation and treatment programmes. This will include patients who may have difficulties in understanding or communicating.
  • To manage clinical risk within the Home 1sttherapy caseload.
  • To be responsible for the safe and competent use of all appropriate equipment.
  • Work collaboratively with the MDT with discharge planning.
  • To comply and uphold standards of the HCPC and the Royal College of Occupational Therapists.
  • To be responsible for maintaining own competency to practice through CPD activities and to maintain a portfolio which reflects personal development.
  • To be responsible for attending and participating in formal supervision, including clinical supervision, in accordance with Trust policies and procedures and the standards of the RCOT.
  • To participate in the staff appraisal scheme as an appraisee and be responsible for complying with your agreed personal development programmes to meet knowledge and competency requirements.
  • To undertake the measurement and evaluation of your work and current practices through the use of evidence-based practice projects, audit and outcome measures in conjunction with the Home 1stLead Occupational Therapist.
  • To be an active member of the in-service training programme by attendance at, and participation in, in-service training programmes, tutorials and individual training sessions.
  • To be responsible for planning own caseload, prioritising workload as situations arise.
  • Acquire an understanding of CNWL-MK clinical and HR policy and procedures.
  • To supervise designated team members and complete appraisals, maintaining accurate documentation.
