Our hospitals and community sites provide a wide range of inpatient and outpatient services, as well as emergency and planned care. The Trust is rated as “good” or “outstanding” in over three-quarters of the services inspected by the Care Quality Commission.

Every year our work includes performing 10,000 procedures in our theatres; bringing seven and a half thousand new lives into the world; carrying out 570,000 visits to patients in their homes or communities and providing emergency care for more than 300,000 patients arriving in our busy Urgent and Emergency Departments.

LGT is a centre for the education and training of medical students enrolled with King’s College London’s GKT School of Medical Education. We are a training centre for nurses, midwives and allied health professionals. We are pioneering new roles that will support the changing needs of our patients and are one of the largest employers of physician associates in the country.

This role is available across four Primary Care Networks within Lewisham:

  • Sevenfields Primary Care Network
  • North Lewisham Primary Care Network
  • The Lewisham Care Partnership Primary Care Network
  • The Lewisham Alliance Primary Care Network

Successful candidates will be employed by one of the above PCNs and aligned to a neighbourhood-based INT. Please indicate your preferred PCN(s) or neighbourhood(s) in your application. Final placement will be based on service need and candidate suitability.

Main duties of the job

We are seeking dedicated and proactive Health and Wellbeing Coaches to join our Integrated Neighbourhood Teams in Lewisham.

The roles focus on delivering the Lifestyle Medicine Service within the Primary Care Networks (PCNs), aiming to improve health outcomes by providing holistic and personalized coaching, particularly in nutrition and lifestyle medicine.

Modality Lewisham provides patient-centred primary care across central and south Lewisham, following the merger of several local practices. We focus on accessible, high-quality services, and personalised health support. As part of the Modality Partnership, we uphold NHS values while improving outcomes for our diverse local community.

Aplos Health PCN is a collaborative network of four GP practices in south and west Lewisham, serving over 50,000 patients. We deliver integrated, community-focused care, addressing health inequalities through innovative services like youth wellbeing clinics and digital hubs.

Detailed job description and main responsibilities

Main Duties and Responsibilities
  • Deliver 1-to-1 and group lifestyle coaching with a focus on nutrition and wellbeing.
  • Support patients in setting health goals and making sustainable lifestyle changes.
  • Manage a caseload and maintain accurate records of progress and outcomes.
  • Work collaboratively with GPs, care coordinators, and community services.
  • Promote service awareness across the PCN and wider community.
  • Collect feedback and contribute to service evaluation and improvement.

Essential criteria
  • • Accredited Nutritionist and PCI-accredited Health Coach.
  • • Experience delivering health and wellbeing coaching.
  • • Strong communication, motivational interviewing, and coaching skills.
  • • Ability to manage a caseload and work as part of a multidisciplinary team.
