Based at Doncaster Older People’s Mental Health Service
The Mental Health Services for Older People offer inpatient, community and day services to people usually over the age of 65, who present with a range of mental health difficulties.
We are looking for an experienced and enthusiastic Community Mental Health Nurse to join the Central Community Mental Health Team, working alongside the registered members of staff in providing the best possible care. The post holder will be an integral member of the multi-disciplinary team and be able to demonstrate considerable previous experience of working in care of older people either in the community or inpatient setting.The Trust offers flexible working arrangements, clinical supervision, development planning and career planning.

- To promote and maintain the highest attainable standard of nursing care in a community setting.
- To screen referrals for the team as allocated by the Team Leader through the Single Point of Access Protocol and cover duty days.
- Perform reassessment Mental Health Assessments
- To maintain close working relationships with referring agencies.
- To deputise for the Team Leader when required.
- To carry a caseload of patients with both functional and organic issues as identified through the assessment process.
- Manage and display a positive attitude towards changes as set out within service requirement literature i.e. 5 Year Forward Doncaster Place Plan
