We are looking to recruit a part time (fulltime considered) registered nurse to work within the Belper Community Nursing team, based at Babington Hospital

Main duties of the job
To manage referrals into our service and provide evidence-based care to support patients in their own home.
This great opportunity includes working alongside peers from nursing teams across the locality, general practice, therapy and our Home visiting colleagues.
To assess, plan, implement and evaluate individualised patient care, promoting self care wherever possible in an efficient and clinically effective way, whilst maintaining privacy and dignity.
To provide advice and support to junior staff in the delivery of high quality care and actively support the District Nurse and Senior Community Nurse in the management of the health care environment.

Education / Qualifications
Essential criteria- RGN with full current NMC registration
- Evidence of recent relevant training
Experience and Knowledge
Essential criteria- Experience of working in the community
- Evidence of research based/reflective practice
- Experience in Long Term Conditions
- Experience in End of Life Care
- Wound Care experience
Other
Essential criteria- Full UK driving licence and own car to use for work purposes or ability to travel during the working day
- Able to work weekend and bank holidays
