More detail about the role The focus will be on immediate EOLC needs; advanced care planning and addressing goals of care; managing non-specific symptoms in multimorbidity and specialist Palliative Care needs.

We would look for this role to interface with the hospital palliative care team, Hospiscare team and community team. In Acute Medicine the role will include working clinically in 3 distinct environments including: Same Day Emergency Care (SDEC) Here you will manage patients acutely that can have their care completed in the same day this includes procedures and investigations Acute Medical Unit (AMU) You will look after patients deemed to need admission having a wide range of symptoms and care needs.

There is direct consultant presence from 8am to 10pm. Ambulatory Care and acute hospital at home This is where patient follow ups and ongoing procedures occur. This is also where we run an MDT clinic for ACP and symptom control. It also allows processes to support patients at home and transitioning onto community palliative care services.

We are excited to develop this further. Within the Emergency Department we would expect in reach and support to the Emergency team for catastrophic events and alternative to admission processes. The post is offered initially for 6 months.
