We are seeking a dedicated and experienced ASW - Senior Social Worker - Independence & Access to join our dynamic team. This role is vital for enhancing the quality of life for individuals in our community by supporting their independence and ensuring they have access to essential services.
The successful candidate will be responsible for assessing clients’ needs and developing tailored support plans. You will collaborate with various stakeholders, including healthcare professionals, community organizations, and family members, to advocate for our clients' rights and well-being.
Additionally, you will provide guidance and supervision to junior social workers, reinforcing the principles of best practices within social service delivery. With a focus on promoting independence, this role will enable you to make a significant impact on the lives of adults facing various challenges, including physical disabilities, mental health issues, or social isolation.

Responsibilities
Conduct comprehensive assessments to identify the needs of clients and develop individualized support plans.
Advocate for clients in accessing resources and services that promote independence.
Collaborate with multidisciplinary teams to ensure comprehensive service delivery.
Provide supervision and mentorship to junior social workers and interns.
Maintain accurate and up-to-date case records and documentation in compliance with regulations.
Engage with clients and their families to establish rapport and trust during the support process.

Monitor and evaluate clients' progress and adjust support plans as necessary, ensuring continuous improvement.

Requirements
Master's degree in Social Work or a related field from an accredited institution.
Minimum of 5 years of experience in social work, particularly in community-based settings.
Current state licensure or certification as a Social Worker.
Strong understanding of social service systems and community resources.
Excellent communication and interpersonal skills to effectively collaborate with diverse populations.
Proven experience in case management and developing support plans for individuals with complex needs.

Ability to work independently and as part of a team, demonstrating leadership skills.

Hours Per Week: 37.00

Start Time: 09:00
End Time: 17:00
Pay Per Hour: £26.50
