Adult Social Worker, Hospital Team, Cheshire West
Role Overview:Pertemps is seeking an experienced Adult Social Worker to support discharge planning and assessment. This role requires expertise in conducting Strengths-Based Assessments, developing care plans, and assessing Mental Capacity. The successful candidate will also contribute to Best Interest Meetings and provide safeguarding support as required.
Key Responsibilities:
- Conduct Strengths-Based Assessments to determine the needs of adults requiring care.
- Develop person-centred care plans to support individuals post-discharge.
- Undertake Mental Capacity Act assessments and participate in Best Interest Meetings.
- Work within a fast-paced environment to facilitate timely hospital discharges.
- Collaborate with multi-agency teams, including NHS professionals and care providers.
- Support safeguarding processes where necessary.
Key Requirements:
- Qualified Social Worker with Social Work England registration.
- Experience in adult social care, hospital discharge planning, and assessments.
- Strong knowledge of the Care Act, Mental Capacity Act, and safeguarding procedures.
- Ability to manage a fast-paced caseload effectively.
- Strong communication, problem-solving, and decision-making skills.
Working Arrangements: Hybrid – mix of home-based working, hospital assessments, and office attendance (Winsford or Ellesmere Port). Primary hospital for assessments is the Countess of Chester, with occasional cover for other hospitals where Cheshire West residents are admitted.
