Description

We are seeking a compassionate and dedicated CSW - Social Worker - Disabilities (Gr 8) to join our team, focused on enhancing the quality of life for individuals with disabilities. This role is pivotal in advocating for the rights and needs of our clients, facilitating access to essential services, and creating individualized support plans to empower individuals and their families.
As a Social Worker specializing in disabilities, you will collaborate with healthcare providers, educational institutions, and community organizations to provide comprehensive support. Your insights and interventions will make a significant impact on the well-being and independence of those you assist.

Responsibilities
Conduct comprehensive assessments of clients with disabilities to identify their needs and strengths.
Develop, implement, and monitor individualized service plans in collaboration with clients and their families.
Advocate for clients' access to services, resources, and opportunities within the community.
Provide counseling and emotional support to clients and their families regarding challenges related to disabilities.
Coordinate with medical professionals, educational institutions, and community organizations to ensure integrated service provision.
Maintain accurate and up-to-date case records, documenting services rendered and client progress.

Participate in multidisciplinary team meetings to discuss client outcomes and enhance service delivery.

Requirements
Master's degree in Social Work (MSW) from an accredited institution.
Current state licensure as a Clinical Social Worker (CSW) or equivalent certification in Social Work.
Minimum of two years of experience working with individuals with disabilities in a clinical or community setting.
Strong knowledge of disability rights and relevant federal and state laws and regulations.
Excellent communication and interpersonal skills, with the ability to build rapport with clients and their families.
Demonstrated problem-solving skills and the ability to work independently as well as part of a team.

Proficient in using case management software and Microsoft Office applications.

Hours Per Week: 35.00

Start Time: 09:00
End Time: 17:00
Pay Per Hour: £26.34
