Key tasks / duties: To provide skilled nursing care under the direction of the Team Leader that is evidence based and best practice. To manage Wellbeing therapy sessions. To co-ordinate daily activity with the Hospice at Home service. To undertake patient assessments in the Wellbeing setting and the patients home, according to individual needs of the patient and their carers.
To contribute to planning, implementation, and evaluation of programmes of care for individual patients and their families and carers. To develop communication skills to promote beneficial therapeutic relationships with patients and their carers.
To attend all mandatory training and take part in education programmes in partnership with colleagues. To liaise with the multidisciplinary team in both the Hospice and the community setting including Palliative Consultant, District Nurse, Social Workers, GPs, and any other agencies involved in patient care.
Support peoples equality, diversity, and rights. Maintain and support the efficient use of physical and / or financial resources. To monitor patients condition and report significant changes to the Team Leader, GP and/or emergency services as appropriate.
Management: Ensure economic use of resources. Ensure a safe environment for patients, visitors, volunteers, and other staff. Participates in the induction of new staff. Assists with the supervision and support of Health Care Assistants, health care students, and volunteers.
Ensure that information on activities and work patterns are submitted in a timely fashion. To take part in the duty rotas to ensure nursing services are covered. To contribute to the monitoring and evaluation of clinical services. Collect appropriate data, i.e. Attendance figures, Safe staffing tools, Patient outcomes and experience for the use of reports and quality evaluations.
Provide an environment that encourages the patient to play an active part in their care enabling them to voice their wishes about current and future care, e.g. the use of advanced care planning tools. Promote a culture that is conducive to the recognition of complex, physical, spiritual, social, and psychological needs and utilise internal referral systems in a timely and appropriate manner.
Provide support, information and advice to patents and carers to enable them to make informed decisions about treatment and care in order to maximise their potential. Act as an advocate to ensure that the patients perspective is heard within the multi-disciplinary team.
Empower individuals to adjust to and manage large scale changes in their lives to maximise their potential within the constraints of their conditions and circumstances. Support the development of new initiatives. Professional/clinical: Maintain up to date registration and revalidation with the NMC and adheres to the professional Code of Conduct.
Accepts responsibility for his/her own professional development. Participate in Clinical Supervision. Promotes evidence-based practice and contributes to research and development activity to enhance patient care. To participate in regular 1-1s and annual performance and development reviews.
To participate in mandatory training sessions. Disseminate information and knowledge gained from attendance at conferences and courses for the benefit of the multi-professional team. Awareness of the legislation regarding registration of the Hospice and guidance laid down by statutory bodies.
Adhere to Hospice policies & procedures relating to own workplace practice. Support the Clinical Services Manager in implementing agreed initiatives, thus improving the effectiveness and efficiency of our services. Person/professional: To act at all times in accordance with the policies and procedures of Rossendale Hospice.
At all times act as an ambassador for Rossendale Hospice to patients, relatives, carers, colleagues, and members of the public. Engage in lifelong learning and attend any training as deemed appropriate. Participate in annual appraisal. Attendance at all mandatory training as specified for the role.
To act as link nurse for specific specialist topic areas. Health and safety: Understand building fire regulations and agreed action to be taken in the event of a fire. Report any incidents/adverse events in accordance with Rossendale Hospice policy.

Understand the principles of infection control and hand hygiene. Take all measures to ensure the safety of staff, patients, volunteers, supporters, customers and visitors to Rossendale Hospice in accordance with Rossendale Hospice Health and Safety Policy.
