About the role
The Review Team is a friendly and supportive multidisciplinary team undertaking annual Care Act Reviews with adults receiving support services.
The role entails taking a strength-based approach supporting people to use preventative services, and to build resilience and achieve outcomes that are important to them, involving the person and supporting them to have choice and control.
This role offers flexible working arrangements, including remote working options. However, applicants must be within commuting distance of Reading as site visits are an essential part of the role.
Key Responsibilities:
Conducting reviews and reassessments of care packages to ensure they meet the needs of individuals in line with Care Act 2014, Mental Capacity Act 2005, and local policies and procedures.Collaborating with service users, families, and multidisciplinary teams to develop and maintain person-centred care plans.
Providing advice, support, and guidance to individuals to promote their independence and well-being.
Maintaining accurate records and ensuring all documentation is up to date.
Working within relevant legislation, policies, and procedures to deliver high-quality care services.
About You:
Previous experience working in Adult Social Care or a similar setting.Strong understanding of the Care Act 2014 and other relevant legislation.
Excellent communication and interpersonal skills.
Ability to work independently and as part of a team.
Strong organisational skills and attention to detail.
