Our short break service based in Grindon Area of Sunderland requires a dynamic and motivated Team Leader with the ability to work in challenging environments, to undertake a lead role in ensuring the care and welfare of our customers who require differing levels of support to meet a range of social and sometimes complex health needs.
The Team Leader is responsible for the co-ordination of staff on shift to ensure the needs of young people and adult are met, give guidance and support to all staff and work closely with the management team to ensure an outstanding service is delivered.Our CQC Registered Short Break Services provide planned breaks to young people and adults who have a range of Learning and Physical Disabilities and Autism.
As a Team Leader, you will undertake a lead role in ensuring the care and welfare of each customer, developing a customer’s quality of life by ensuring they have a positive Short Break experience whilst providing their carers with the peace of mind that their loved one is receiving the highest standards of care.This may include supporting a customer with personal or health care tasks, administering medication, money management, access community activities or provide emotional and/or behavioural support whilst developing and maintaining positive relationships with their parents and carers.
In addition, you will coordinate and contribute to the ongoing development of care plans and quality assurance, ensuring the seamless transition from home to short breaks ensuring person-centred care and support is given to customers and their families.
You will need to work effectively as part of a team, within a multi-agency framework, ensuring you provide support to team members, as well as care and support to customers to achieve their maximum level of independence through appropriate physical, emotional and psychological support including, intimate personal care, support with a range of health needs, practical tasks, appropriate communication and community involvement.
We are looking for a Team leader who is motivated to provide and promote high standards of customer care along with a commitment to their own personal development. You will be supported on your journey via training, supervision, and appraisal to ensure you receive the support designed to meet your personal needs.You will be required to assess and manage cases with a degree of complexity and risk, taking action to coordinate resources and support to maintain or improve the wellbeing of customers, which also includes providing personal and practical interventions to care and support in line with individual care plans, including liaising with families and health and social care professionals.
The new Government Regulations, that require all persons working in or entering Care Quality Commission (CQC) regulated care homes to be fully vaccinated, apply to this role. The successful post holder will be required to provide evidence they have been fully vaccinated against COVID-19 (unless medically exempt) as a condition of employment.
