Responsibilities:
Develop and deliver solutions to ensure that Kingston is demonstrably maximizing the effectiveness, impact and quality of its reablement offer, to prevent, reduce and delay the need for long term care
Develop a proposed new service model to implement over medium term
Develop an update set of detailed recommendations and implementation plan to deliver improvements to the Reablement services in 2025/26Significant levels of engagement with people who access services, front-line works, social workers, health partners and commissioners
Requirements:
Good understanding of adult social are, who has experience in engaging with people from a range of roles and backgrounds, analysing data and developing new service models/recommendationsUnderstanding of commissioning in adult social care
Experience in reviewing services and recommending improvements/designing new service models
