The post holder will work with the Prevention Lead Midwife overseeing Tobacco Dependence, and other team members in providing individualised stop smoking support to women and families in a flexible and innovative way. This post will contribute to tackling inequalities in health through promoting and supporting people to stop smoking.

It focuses on engaging with people within communities and offering them practical support to change their behaviour to achieve their own choices and goals

Specifically, the post holder will:

  • Provide intermediate stop smoking support throughout pregnancy and the post-natal period, using the support of interventions such as CO monitoring and Nicotine Replacement Therapy.
  • Work with the specialist Prevention Lead Midwife to deliver stop smoking support, undertake delivery of the Smokefree pregnancy incentive scheme with those women and families who meet the criteria, along with administration of the smokefree pregnancy data collection and collation.
  • Promote the general health and wellbeing of people who use the service through offering healthy lifestyle advice and support.
  • Seek out opportunities to develop and implement stop smoking services in a range of health and community settings.
  • Work with minimal indirect supervision operating within the assessed level of competence and being accountable for all aspects of own work.
  • Be part of public health initiatives as part of the prevention team

Care/Service Provision

To plan and deliver stop smoking advice and support sessions, including advice on stop smoking medications, working within recommended protocols and guidelines. This may involve individual, telephonic, on-line and group work sessions in a variety of settings and venues.

Ø Identify and approach women who are eligible for tobacco dependence conversations and treatment in a variety of different settings. These may be in a clinical area, virtually, or in a person’s home. Follow women through their engagement with the service to the location of their care provision in order to integrate care.

Ø Develop individually tailored support plans including recommending treatments based on assessed needs.

Ø Provide general health, lifestyle advice and support to people using the service e.g. healthy eating, physical activity, stress management. This will include individual and group-based interventions, signposting and referring to other health, community, and voluntary services.

Ø Participate in educating pregnant women about the benefits of stopping smoking in pregnancy and give support to those women wanting to quit. Promote smoke free homes and support and advise women and families about passive smoking.

Ø Deliver Carbon Monoxide (CO) monitoring to a high standard and engage in the data collection and collation of CO rates in pregnancy and post-partum.

Ø Engage with the Smokefree pregnancy incentive scheme, keeping accurate records and enable good data collection. Maintain accurate up to date and comprehensive records at all times.

Ø Ensure that the relevant equipment is provided and maintained on an agreed schedule. Report any failings of equipment or other environmental factors to the Prevention Lead Midwife.

Ø Respond effectively to referrals within stated timescales – self-referrals from the individuals, and those from colleagues / other workers / GPs.

Ø To be involved in the monitoring and evaluation of client feedback, offering recommendations as part of the team to improve services.

Ø Enable utilisation of the smokefree pregnancy platform, for recording patient data, outcomes, review own personal performance against programme KPI’s.

Ø Ensure the lone worker policy is always followed, undertaking adequate communication with the team before and after visits.

Ø Be aware of how to report incidents, concerns or complaints and escalate any concerns or complexities to the Prevention Lead Midwife.

Working as part of a team:Ø Alert the specialist midwives or appropriate line manager to any issues in work (including concerns about individuals or work in the community) and maintain confidentiality.

Ø Contribute actively to open discussions of how local services can be improved, supporting improvements within the local community, and demonstrating an understanding of the barrier’s individuals are experiencing.

Ø Play an active role in the development of the Smoke Free service targets and work collaboratively with team members to share best practice and information to achieve these.

Ø To maintain good working relationships with all levels of staff

Ø To liaise with others in the health and social care community in order to promote the stop smoking service and encourage appropriate referrals.

Ø To support the delivery of tobacco dependence training to the multidisciplinary team.

Managing self:

Ø Undertake relevant training to maintain the skills and knowledge as necessary through participation in continuous development.

Ø Work with indirect minimal supervision from the Prevention Lead Midwife, always operating within the assessed level of competence. Clear professional boundaries and limitations will be discussed and monitored by the specialist midwife.

Ø Work within the agreed and relevant policies and procedures and know how to access them.

Ø Seek advice and support as and when appropriate.

Ø Plan own activities, sessions and plans to meet the needs of individuals within the service.
