We have a fabulous opportunity available for you to join our awesome team as our Support Coordinator. We support our brilliant customers who have complex mental health support needs. We help them live independently in the community in a flexible and bespoke way to support them to meet their goals and aspirations.

The opportunity is in our community and floating support service which we call STEP (Supporting Tenancies, Enabling People) with our customers who live in a community-based setting in the Sunderland and Washington areas.
The STEP service is successfully being developed across the Northeast and is still in its infancy in our area. This is an exciting opportunity to get involved in developing the service and supporting it to evolve.

Typical day as our Support Coordinator
  • Creating support plans with your customers and working together as a team to ensure the smooth delivery of support.
  • Supporting our customers with complex mental health needs, learning difficulties and/or dual diagnosis. Attending MDT's, risk planning meetings etc to enable us to best support our customers.
  • Helping customers with daily living activities such as making meals, shopping, budgeting, maintaining their tenancy and moving onto independent accommodation if they are able.
  • Carrying out risk assessments, support planning, goal setting and regular reviews.
Cuppas, chats and catch ups. There’s no rushing from one customer to the next here! Plenty of time to collaborate with colleagues too

  • You’re passionate and motivated to help vulnerable people make positive and lasting changes to their lives. You’ll impress us with your ability to work on your own initiative, positively influencing people from all different backgrounds.
  • We help change our customer’s lives for the better. You’ll play your part by assisting in the delivery of tasks that are based on our customers personal support plans, their needs, and aspirations. You’ll work closely with external agencies and professionals to assess and engage support for our customers.
  • You have experience in safeguarding, risk assessments, confidentiality, support plans, alongside working collaboratively.
  • If you’re not the biggest fan of technology, that’s not a problem to us. You do need to know that we are quite a high-tech company and most of our application process and mandatory learning are online. However, we’re glad to support you if you need help.
  • To make sure our customers are well supported, it’s important that you’ve had a job in a similar environment before. Don’t worry, we won’t drop you in the deep end, we’ll help to you understand how things work at Home Group!
  • As this is a community-based job, you need to be able to drive, as well as having access to a vehicle insured for business purposes (we pay your expenses to cover this use!)

Due to limits on our current certificates of sponsorship, we are not able to offer sponsorship to external & internal candidates for this role. This remains under regular review.

You’ll join Danielle and our mental health outreach team, we’re super passionate about what we do! We’re really proud of our service and the difference we make. We’re a lovely bunch if we do say so ourselves and as well as supporting our customers, we support each other too!

Job details
  • You will work on a rota basis, Monday-Sunday.
  • Where we’re flexible with you, we do need to meet the needs of our customers. That may mean adapting your start or finishing times on occasion.
  • You’ll be able to use technology for creating and updating support plans, complete online learning and to collaborate with colleagues.
  • You’ll need an Enhanced DBS check done and we pay for that.
