Other duties include supporting the Social Care team working closely with another coordinator and being able to cover other coordinator roles when required We are an integrated service, therefore you will be expected to support and cover other teams as needed.
The posts are currently in Poltimore Building, Exeter Community Hospital and Osprey House, Sowton Industrial Estate. Central point of contact for the local multi-disciplinary activity, including the facilitation of the core group function. Co-ordination of the regular (weekly) core groups, taking minutes and noting core group actions.
Preparation for core group discussions, check patient history and involvement with other agencies. Provide a high-quality customer service function in co-ordination of contact and outcome information. The provision of timely and effective co-ordination of contacts for the local Health & Social Care Team, working closely with other parts of the wider community teams.
Identify urgent assessments that need action, alert and liaise with local Social Care Team Managers / Community Nurse, Urgent Community Response and Therapy Managers. Effective signposting of contacts to other agencies or sources of help as appropriate.
Provide feedback to referrers about the outcomes of contacts and assessments in accordance with the agreed quality standards. Maintenance of IT based information systems and responsibility for key performance data. For NHS and DCC. To ensure the IM&T requirements for recording team activity are adhered to in collaboration with other team members.
To provide key performance/activity data within the local Exeter Community teams. To be trained and in the use of EPIC and CF6 along with other relevant IT programmes. To record, send and store information in line with the Data Protection Act and Caldicott principles.
To be able to plan and respond to work load according to operational priorities To participate in the induction of new staff to the cluster team. To take part in regular performance appraisal. To undertake any training required in order to maintain competency including mandatory training.
To supervise staff The role needs someone with a strong administrative background who is well organised with excellent problem-solving skills. You should also demonstrate excellent interpersonal, and prioritisation skills due to the high-volume workload with frequent interruptions.
Training and any opportunities to develop are supported. You will be expected to communicate across a wide variety of contacts, good communication skills with an excellent telephone manner are essential since you can be dealing with distressed patients.
The role involves regular communication with clients, families, clinicians (from a wide range of disciplines) and the voluntary sector requiring strong customer care skills and a professional manner. You will require high levels of concentration due to the work involved along with excellent problem-solving skills.
You should be a good team player, supportive, flexible and willing to offer help to your colleagues in other teams as necessary. You should be resilient and able to cope with a busy office and a large workload where change is constant. A strong administrative background is needed since you will be expected to set up systems which assist the clinical team and produce process notes to ensure consistency.
You should be confident and have excellent IT skills this is a rewarding role helping a wide range of people. Flexibility and the ability to deal with challenging situations in a professional and confidential manner will be required as will accuracy.
You will need to travel to various GP practices, other community sites and attend training courses. This is an interesting role supporting Social Care and will enable the postholder to work across NHS and DCC to gain considerable experience and open other career opportunities.

Working Pattern: 37.5 hours per week, Monday to Friday 9am to 5pm, currently but could be asked to work occasional weekends.
