An exciting opportunity has arisen at the Copeland Community Rehabilitation Team for an enthusiastic Band 4 Assistant Practitioner to join our dedicated team. A great opportunity to try out community ICC working.

This varied role will involve working with community patients, working on their rehabilitation goals, as well as completing rapid response visits with an emphasis on supporting discharge and admission avoidance.

The role also involves working with our in-patients to carryout rehabilitation tasks along with therapy activities where appropriate.

The candidate would need to either hold the band 4 Assistant Practitioner qualification or demonstrate that they would be able to meet the requirements to undertake the Assistant Practitioner qualification at the next enrolment opportunity.

As an assistant practitioner, you will be part of a multidisciplinary team including occupational therapists and physiotherapists working along side other health and social care professionals to develop the integrated services ensuring that patient care is delivered to the highest possible standard.

The post holder will implement care for patients in the community and other setting, observe the patients condition, providing interventions with a rehabilitation therapy focus and evaluating outcomes.

Flexible working, five days per week, including weekends and bank holidays will be required to meet the needs of the service.

Main duties of the job
  • Manage a community therapy caseload, completing rehabilitation treatment plans and treatment sessions.
  • Working closely with Occupational Therapists, Physiotherapists, District Nurses and wider ICC team members on joint treatments and visits.
  • Supporting admission prevention through rapid response visits.
  • Following the D2A model in both community hospital and acute settings to support discharges.
  • Completing a variety of assessments including but not limited to; personal care, kitchen assessments, mobility assessments, falls and environmental assessments.
  • Assess and provide equipment and adaptations
  • Work with patients to maximise their rehabilitation potential which includes providing an actioning therapeutic activities.

Essential criteria
  • Have a Foundation Degree – Health and Social care, (Assistant Practitioner route) or equivalent. Maintenance of professional portfolio
  • Level 2 Maths, Level 2 English or equivalent, Care Certificate

Knowledge and Experience

  • Significant experience of working within a healthcare environment.
  • Experience of caring directly for patients, relatives and carers
  • Experience working with in a multidisciplinary team
  • A basic understanding of the use of IT
  • Understanding of and adherence to boundaries of practice and professional conduct
